How to start a medical practice

  1. 1

Form a professional entity and EIN

Our office forms your professional corporation or professional LLC in the state where you practice and gets the EIN. Most states require owners to hold the matching license.

  2. 2

Register the entity with your state

Some states require a professional entity to register with the medical or dental board before it practices. Check your state board's rules and renewal dates.

  3. 3

    Get NPI and payer enrollment

Apply for a group NPI, then enroll with Medicare through PECOS, your state Medicaid program and private insurers. Prescribers need DEA and any state controlled substance registration for the new location.

  4. 4

    File local licenses

Check for a city or county business license at your office address. Dental and imaging offices usually register x-ray equipment with the state radiation control agency.

  5. 5

Insure the practice and run payroll

Our agency quotes malpractice, general liability, property and workers' compensation. Our office runs payroll and keeps your books in QuickBooks Online.

Can a doctor or dentist form an LLC?
In most states, yes, through a professional LLC or professional corporation, and some states allow a medical corporation. A few states limit LLCs for licensed professions. Owners must usually be licensed in the profession, and some states require the entity to register with the licensing board. Our office forms the entity in the state where you practice and gets the EIN.
Should my medical practice be an S corporation?
Many practice owners elect S corporation tax treatment so part of the profit is paid as distributions instead of wages subject to self-employment tax. You must pay yourself a reasonable salary through payroll. Our office reviews your expected income, files the election, and runs owner payroll so the setup holds up.
What do I need before I can bill insurance?
You need a group NPI for the practice, an individual NPI for each provider, and enrollment with each payer, such as Medicare through PECOS, your state Medicaid program and private insurers. Credentialing can take months, so start early. Our office sets up your entity and EIN first, since every payer application asks for them.
What insurance does a new medical or dental practice need?
Practices usually carry professional liability, often called malpractice, for each provider, plus general liability, property coverage for equipment, cyber coverage for patient data, and workers' compensation once you hire. Hospitals and payers ask for proof of malpractice coverage during credentialing. Our agency quotes these together for your practice.
